Who is a Care Manager?

Care Managers are nurses who will help you or your loved one get the medical care or other help needed to live a happier, healthier life. The Care Manager will ask questions about the patient’s health and then use the answers to work with your Medical Home provider to create a Care Plan that will facilitate a healthy outcome. The Care Manager helps patients find resources or services in their area which would be of help. The Care Manager will stay in touch with you and your Medical Home Provider to make sure that you have all the medical care necessary, as well as other help, in order to live a more healthy life.

How do I learn more about my illness?

Your Care Manager is there, ready to help you learn more about your illness. Remember: Knowledge is Power… and the more you know about your illness, the easier it is to get healthy. You will be sent information about your health condition. The information is easy to read and once you understand it, you’ll be better equipped to make the right health choices for yourself. But you won’t need to do this alone, your Care Manager will set an appointment with you to help you every step of the way. So, when the Care Manager calls and asks if you want help with your health care, just say “Yes” and you’ll be glad you did!
